SPONSORSHIP FORM AND ADVICE

Although Pants in the Park is primarily about raising awareness of all prostate diseases, we would be extremely grateful for any additional funds, large or small, which you are able to raise. Here are a few suggestions as well as a guess my time game and sponsorship form.

*  Build a free sponsorship page – personalise your own page in minutes with photos and email it to your friends
*  All donations come straight to us – saving you the hassle of collecting the sponsorship payments, Justgiving processes your online donations and sends the money to us.

If you need a sponsorship form you can download one here. Please feel free to photocopy the sponsorship form if you require more copies.

Paying money in
We would prefer to receive sponsorship payments in one lump sum, therefore saving on administration. However, we do accept part payments. Please ensure you clearly indicate on your sponsorship and payment forms whether your payment is final or part. All cheques should be made payable to 'Prostate UK' and be sent together with sponsorship forms to Prostate UK, 10 Northfields Prospect, Putney Bridge Road, London SW18 1PE. Do not forget the sponsorship form because without it we cannot claim Gift Aid.

Unisex T-shirts
If you would like to receive a free Prostate UK T-shirt (available in small, medium, large and XL) we will be pleased to send it to you. T-shirts will also be available at the event. Although the T-shirts are completely free of charge – we would be extremely grateful for a £5 donation to cover the cost of production and postage. Go to our on-line request form here.
